Referrals: 

Our referral staff will set up any referrals that your physician feels are necessary for your medical care.  They will check your insurance coverage and forward all necessary information to the
specialists office or testing center for you.  They will contact you in 2-3 business days after your appointment with our physicians to let you know when your appointment is, where the office or testing center is located, and any special instructions that you need may need to know regarding your appointment. Most insurance require a referral from your primary care
physician in order to be covered by your insurance. Check with your specific carrier for details.

Medical Records Release From FPC: This form can be used to transfer your medical records to another physician such as a
specialist’s office,  or for your own reference.  Please read the
release carefully as there are some instances in which there are charges for copying of medical records.
