Appointment Information:
For maximum convenience and efficiency, our office is on an
appointment system. We make every effort to honor appointments as scheduled and ask your cooperation when our schedule is occasionally disrupted by an emergency or unforeseen delay. Routine appointments may almost always be scheduled with the physician of your choice. If your primary care physician is unavailable, or in the case of an acute illness or injury, you will always have access to the same high quality of care from any of our providers on duty.
Cancellations and Missed appointments: If you must cancel an
appointment, a twenty-four (24) hour notice is required. A $25.00 charge will be added to your self-pay account for any
appointments not cancelled with the proper notice. Missed
appointments that were not cancelled (No Call No Show) are subject to a $55.00 charge on your self-pay account. These charges for late
cancellations and missed appointments will not be billed to your
insurance and must be paid in full before scheduling your next appointment.
